A diverse collection of malignancies known as hepatobiliary cancers have minimally effective standard treatments for advanced disease, and these treatments change gradually over time. The effects of immune-based treatments have not yet been felt, much like the majority of gastrointestinal malignancies, with a few significant exceptions. Despite this, the etiological background of hepatobiliary cancer—which overlaps with inflammation in nearly every known cause or related factor—offers a strong indication that these strategies may have an effect on this group of disorders. This review aims to highlight the therapeutic prospects in immune-based therapies now entering the clinic or those that are soon to do so by placing the management of hepatobiliary malignancies in the perspective of their inflammation-based aetiology. Hepatocellular carcinoma or biliary tract carcinomas, which are caused by the intra- and extra-hepatic biliary ductal system, are primary malignancies that originate in the liver.
